What are the main requirements for the UMD immunization form?
2 MMR Vaccines
- First dose: After 1st birthday
- Second dose: After age 4
- If given as an adult: Must be at least 4 weeks between doses
TDAP
One dose is given at age 11 or older
Meningitis
- For incoming students, all undergraduate students will need a meningitis vaccine or waiver
- One dose given after age 16
- Must be given within the past 3 years
- This vaccine can be waived by completing the waiver in Section C
TB Risk Screening
ALL students must complete an online questionnaire.
If any questions are answered “YES”, you are required to have a Quantiferon Gold or T-Spot blood test. This test MUST be performed in the United States.
Where can I find my vaccination records?
The Health Center offers all vaccines that are required. Please call for pricing at 301-314-8114.
MMR titers: A titer is a laboratory test that measures the presence and amount of antibodies in blood. A titer may be used to prove immunity to disease. If you cannot find records of your MMR vaccine, your medical provider can perform titers to verify positive immunity. You must provide a copy of the lab report for verification.
How to find your records:
- Ask parents or other caregivers if they have records of your childhood immunizations.
- If you received your childhood vaccines in the state of Maryland, you may be able to access them through the state's immunization registry. Visit MyIR Mobile to get started.
- Try looking through baby books or other saved documents from your childhood.
- Check with your high school and/or college health services for dates of any immunizations. Keep in mind that in some cases, records may only be kept for 1-2 years after students leave the system.
- Check with previous employers (including the military) who may have required immunizations.
- Check with your doctor or public health clinic. Keep in mind that vaccination records are maintained at the doctor's office for a limited number of years.
- Contact your state's health department. Some states have registries (Immunization Information Systems) that include adult vaccines.
Where do I submit my Immunization Form?

When I try to upload my form on the MYUHC Portal, there is an error message. What do I do?
- If you are getting a message that says, 'We apologize, but we were unable to locate your record. Please contact the health center.’ please give us a call at 301-314-8114.
- Also, you can try to clear the cache and cookies from your internet browser.
Do you accept PPD skin tests if I answered "Yes" on the Tuberculosis Risk Screening?
No. The University of Maryland will NOT accept the PPD skin test instead of the Quantiferon Gold or T-Spot test.
Do I still need to submit an Immunization Record if I am taking all Online Classes?

I recently had a TD vaccine. Am I still required to have a TDAP vaccine?
Yes. The University of Maryland requires one dose of TDAP given at age 11 or older. TD protects you against tetanus and diphtheria, while TDAP protects you against pertussis (whooping cough) as well as tetanus and diphtheria.
If my Quantiferon Gold Blood Test result comes back positive, what do I do next?
If your Quantiferon Gold Blood test result is positive, this means you have had an exposure to tuberculosis (TB) in your lifetime. To make sure you do have active TB, you will need to have a chest x-ray. This can be done at the UHC or another facility in the community. You must attach your x-ray report (in English).
If your chest x-ray is normal, your doctor should discuss treatment for latent (inactive) TB. You must provide documentation of this review, even if you decline treatment. The medical provider must complete Section D of our immunization form completely. Please make sure the medical provider completes the clinical evaluation and Treatment for Latent TB in Section D. The medical provider must also sign and stamp the bottom of Page 3.
What if I need a medical or religous exemption from a vaccine?
Students may request a religious exemption from immunization requirements if vaccination conflicts with their bona fide religious beliefs and practices. Religious exemption requests may be submitted through myuhc.umd.edu by clicking on Required Immunizations and Forms. Select the vaccine for which the exemption is being requested, then scroll to the bottom of the page and click Request an Exemption.
During an epidemic or disease outbreak declared by the Maryland Secretary of Health (or designee), exempted students must either be immunized or refrain from attending classes and campus activities until the outbreak has ended.
Students may also qualify for a medical exemption by submitting a written statement from a licensed physician or local deputy state health officer indicating that a specific vaccine is medically contraindicated. The statement must specify whether the contraindication is permanent or temporary. For temporary exemptions, the physician must include the date the student can be vaccinated, and the student must provide documentation once the immunization is completed. Medical exemption requests and supporting documentation may be submitted through myuhc.umd.edu by clicking on Required Immunizations and Forms. Select the vaccine for which the exemption is being requested, then scroll to the bottom of the page and click Request an Exemption.
During an epidemic or disease outbreak related to the student's immunization status, medically exempt students may not attend classes or campus activities until the outbreak has resolved.
