Located on the ground floor of the Health Center, room 0106, the Occupational Health Unit provides a number of work-related services to employees. Occupational Health works closely with the Department of Environmental Safety, Sustainability, and Risk to create and maintain a safe and healthy work environment.
Before you visit
Supervisors must complete the form below to initiate an Occupational Health referral for an appointment.
- Billing: Responses provide the necessary billing information to authorize the visit.
- Scheduling: Once submitted by the supervisor, the Occupational Health team will coordinate directly with the employee to schedule their appointment.
Please note:
This form is not required for reviews regarding Animal Handler or Respiratory Clearance.

Workers’ Compensation
Workers’ Compensation is the Maryland State statutory insurance program that provides medical and hospital treatment, partial income replacement benefits, and additional benefits to employees who sustain work-related injury or illness. Fitness for duty
Fitness for duty refers to the physical and/or mental readiness of an employee to perform the essential functions of his or her job. A fitness for duty examination may require psychological, medical and or substance abuse assessment.
