Occupational Health
Home Occupational Health

Located on the ground floor of the Health Center, room 0106, the Occupational Health Unit provides a number of work-related services to employees. Occupational Health works closely with the Department of Environmental Safety, Sustainability, and Risk to create and maintain a safe and healthy work environment.

Before you visit

Supervisors must complete the form below to initiate an Occupational Health referral for an appointment.

  • Billing: Responses provide the necessary billing information to authorize the visit.
  • Scheduling: Once submitted by the supervisor, the Occupational Health team will coordinate directly with the employee to schedule their appointment.

Please note: 

This form is not required for reviews regarding Animal Handler or Respiratory Clearance.

DOT Physicals

Schedule your DOT physical at the University of Maryland Health Center. Find appointment scheduling details, patient portal requirements, and necessary forms for UMD employees and Shuttle-UM drivers.

Please call 301.405.3153 or 301.314.8184 to schedule your DOT appointment.

You will need to schedule a separate appointment with the lab for a urine test. If this is your first DOT physical for Shuttle-UM, you will need to schedule an additional appointment for drug testing.

Prior to your appointment with the provider, update your “Health History” in the Patient Portal. In addition, complete and sign section 1 of the DOT Medical Examination Report Form:

  • Driver Health History (pages 1-2)
  • CMV Driver's Signature (page 2)

WHEN YOU ARRIVE FOR YOUR APPOINTMENT, PLEASE BRING THE FOLLOWING ITEMS:

  • Completed DOT Medical Examination Report Form (pages 1-2)
  • Driver’s License
  • Drivers with vision problems must bring their eyeglasses or contacts
  • Drivers with hearing problems must bring their hearing aids

Drivers with psychological disorders (i.e. ADHD, depression, bipolar disorder, etc.) must bring a completed CMV Driver Medication Form from their mental health professional (psychiatrist or psychologist)

DRIVERS WITH DIABETES MUST BRING THE FOLLOWING:

  • A copy of their most recent blood test results that includes their Hemoglobin A1C (HgA1C) level (within the last 3 months).
  • Drivers that require insulin to treat their diabetes must also bring The Insulin-Treated Diabetes Mellitus Assessment Form that has been completed by their treating clinician within forty-five (45) days of your DOT appointment. In addition, these drivers must also bring records of their preceding three (3) months of ongoing blood sugar logs.

The Occupational Health Unit provides several work-related immunizations for faculty and staff. For an appointment call 301.405.3153.

Influenza

Influenza spreads throughout the US every year, usually between October and May. Anyone can get the flu. There are many flu viruses, and they are always changing. Each year a new flu vaccine is made to protect against the influenza viruses believed to be likely to cause disease in the upcoming flu season. Even when the vaccine doesn’t exactly match these viruses, it may still provide some protection. Influenza vaccine does not cause flu. Vaccination is recommended  every flu season for everyone 6 months and older.

Rabies

Those who work with potentially infected animals may be at higher risk for Rabies exposure. Vaccination is recommended to help prevent rabies if an exposure happens. If you are at higher risk of work-related exposure to the rabies virus:

  • You should receive 2 doses of rabies vaccine given on days 0 and 7 before starting work if you have no history of prior immunization.
  • Depending on your level of risk, you may be advised to have one or more blood tests or receive a booster dose within 3 years after the first 2 doses. Your health care provider can give you more details.

Rabies vaccine can prevent rabies if given to a person after an exposure. If you experience a work-related exposure, seek treatment immediately at the Health Center or the closest emergency room. If it is after hours, go to the closest ER.

Hepatitis B

If your position has been identified as one in which there is exposure to blood and body fluids, we recommend you complete the Hepatitis B vaccine series (two doses 30-days apart) for protection from this blood-borne infection. For an appointment call 301.405.3153.

TDap & Td

Tdap vaccine can protect us from Tetanus, Diphtheria and Pertussis. All three are serious diseases caused by bacteria. Although rare, depending upon the nature of your work and occupational hazards, you may be at risk for exposure to the bacteria that causes Tetanus through cuts, scratches, wounds, or severe burns. It is important to know when you were last vaccinated and to report injuries to assess if a booster is needed. An updated Tetanus booster may also be a requirement for your work.

Workers’ Compensation

Workers’ Compensation is the Maryland State statutory insurance program that provides medical and hospital treatment, partial income replacement benefits, and additional benefits to employees who sustain work-related injury or illness. Fitness for duty

Fitness for duty refers to the physical and/or mental readiness of an employee to perform the essential functions of his or her job. A fitness for duty examination may require psychological, medical and or substance abuse assessment.
