  • If you do not have a referral, you may want to make an appointment with a medical provider in the Health Center or with your personal physician.
  • If your referral is past the 60 days since it was written, please call your doctor for an updated referral.

Health Insurance

For students covered by the Student Health Insurance Plan (SHIP), charges are fully covered.

For any other health insurance company, we are currently working with them to accept their plan.  All charges are the patient’s responsibility. You should call the insurance company to determine whether UHC physical therapy services are eligible for reimbursement.  Upon request, we do provide a “Walk Out Statement” for you to submit to your insurance company for potential reimbursement.

For students, if your insurance does not cover costs at this time, you may still be seen and charged as Self-Pay with a student discount rate.  Office visit discount rate applies to registered students only. Fees vary based on the type of visit and procedures done during the visit.  Upon request, we do provide a “Walk Out Statement” for you to submit to your insurance company for potential reimbursement.
